Dr. Puskar has historically worked with most major insurance carriers.

We will work closely with you and your carrier to ensure coverage prior to appointments.

Currently, State of Mind is in Network with:

• University of Pittsburgh Medical Center (UPMC) insurance

• Highmark

• Medicare

If your carrier is not on the list we can use Out of Network benefits or attempt a single case agreement.

Private Pay

Dr. Puskar's private pay fees are generally consistent with insurance reimbursement and other providers in the region.

For a standard therapy appointment, the fee ranges from $150-200 depending on the length of session.

The cost for a neuropsychological assessment varies depending on presenting question, testing demands, and length of report. One could expect to spend 3-10 hours with direct on site assessment and Dr. Puskar can spend additional hours reviewing records and formulating the report.

Given that each assessment is tailored to each individual, an accurate estimate is difficult to provide. In general, assessments can range from $600 -$2000.

We can provide a more accurate estimate during an initial phone consultation that fits your financial needs. You will never get a surprise bill.
